Delegation of Powers by Parent
This form allows a temporary one-year delegation of powers by a parent or guardian to a 3rd party they have authorized to raise their child.
All in-person legal clinics are temporarily suspended due to COVID-19. We are working to find alternative methods to hold these clinics and will update our calendar when that information is available.
